I hope this is a dumb question; I have searched (and searched) and can't find my issue anywhere. 
Playing with MacOS recently I like its LaunchPad, with the "LED" illumination showing that a Launchpad program is active. I've since noticed on my W7 PCs that Windows Taskbar shows two instances of a program icon, one in the icon's original position on the left side of the taskbar (un-highlighted), and another "glassy" (highlighted) one to the far right of the icon set showing the active instance of the program.
I have selected the "Always Combine" option in taskbar Properties, shouldn't there only ever be a single appearance of a program's icon in the taskbar?
EDIT: It seems that not every program does this, i.e. the two programs we use the most, Thunderbird and Firefox, create a new instance on the rightmost side of the taskbar, even if they ARE ALREADY on the rightmost side of the taskbar. Other progs e.g. Picasa behave as expected--launch the program somewhere in the middle of the taskbar icons, and that program icon is highlighted in-place.
