Video hardware Error

When i'm playing games like cs source, cod4 or using certain software from 10 minutes to 2 hours, the image of the screen suddenly freezes, the black screen appears for a few seconds, video driver recovers, video memory controller load locks up at 100%, the performance of a pc decreases a little bit and windows reports me this http://img101.imageshack.us/img101/9271/16759273.png. This error has been persisting since i assembled my pc six months ago. It really bothers me.

I have tried so far:
Updating video drivers
Running memory test (8 passess, duration 7:47 = no errors)
Plugging video card in another PCI-E socket
Plugging monitor cable into another DVI connector.
Formattting HDD

Can anybody help me solve this issue?

If you are able to build a computer, you know everything points to memory or graphics card or driver.

If you made the memory test with the built in memory diagnostics (it is very inaccurate), try with this: Guide to using Memtest86+

Hum...Interesting..I got this error too, long ago (I use laptop, not PC)
I have ATi Raedon 3400, I remember I were playing some online game and suddenly the game freezze-->black screen-->have to turn off the game by clicking "X", then the computer decrease speed a bit too and the notification appear "ATi has recoverd from smt...". Now it doesn't appear anymore,..:confused:
I tried to do all the thing, update everything...kind of mess up with my computer:
-Re-install the Window
-Update newest BIOS update
-Update power manager driver
-Resolutions are the same between games and the video card utility setting

Please follow what Richard said, I don't see any reasons from my list above, just wanna share to u if it looks like your problem

I used exactly the same memory test as you mentioned "memtest86+"(burnt it to cd and ran it using boot menu during post screen). It passed without any errors 8 times. It seems DDR3 modules are fine.

"freezze-->black screen-->have to turn off the game by clicking "X"" In my case, once the black screen appears and disappears, the games don't hang up, they keep running.
Maybe a motherboard with current BIOS cant process graphics card's memory correctly or there is a defect...

Next time it happens, type event viewer in search and look for errors.
