I wouldn't use it unless your machime has a specific problem and you know the fix that's needed. Even then I'd make a system image backup or at least a system restore point before going ahead.

Tweaking.com usually has reputable tools and utilities. It's not a "do all" program but allows the user to check areas to repair. I suspect it simply changes settings in the same way a user would.
As suggested, I would make a backup of the system before using it.

I have seen people have success with it, but other then image backups mentioned, I'd also list it as a last resort thing. If your next step is a reinstall, then worth a shot
