WMP12 Keeps Adding Same Files???

As I go thru my library, I occasionally notice that I will have doubles of a lot of my files and go through each one and delete the extras. For some reason now there are way to many folder this has happened to for editing each and every one.

I have two questions: why does this do this and how can I fix now that there are way too many to do individually?
