See my post above. The program works great (you can even have multiple control setups saved for different games). Yeah, it's 10 bucks, but that 10 bucks will get you free updates as long as you remember your username and password.
Also, I find it a bit easier to install XBCD and then enable test mode in the Administrator Command Prompt permanently:
Bcdedit.exe -set TESTSIGNING ON
