You might notice that there is no attached DMP file. There is a reason for this, so keep reading, please.

I have done a fair amount of 'self-troubleshooting' for the past few days on this issue with seemingly no concrete answer in mind. However, with my limited knowledge, I believe I have narrowed down the issue to a few possibilities:

  1. System Corruption
  2. Defective/Missing Drivers (I have sought to remedy this possibility, though the problem still persists, therefore I find this unlikely.)
  3. Malware
  4. Defective Hardware

HOWEVER, this is coming from a person that has limited knowledge in this field, and I stress that I am not certain on the correct diagnosis.

Synopsis:

  • The crashes appear only occur during above-average system load--i.e, when playing video games.
  • Windows appears to function normally during normal user operation--i.e, web browsing, forum posting, and using programs such as TS3 and Discord.
  • I have have been receiving BCCode 3b (SYSTEM_SERVICE_EXCEPTION) almost exclusively, until the most recent crash gave me code 19 (BAD_POOL_HEADER).
  • The DMP files found in the system root have been seemingly deleted, despite me instructing Windows to NOT overwrite existing files. Note that DMP files which have been most recently been generated have also been deleted. This is why there is no attached DMP file.
  • I have updated all drivers, and have completely reinstalled my GPU drivers.
  • This problem has been persisting for around three days.
  • These crashes during gaming sessions happen at random, as in; there is no consistent allotted time to crash.

Any help or guidance would be greatly appreciated, thank you for reading.

PS, If a DMP file is of paramount importance when troubleshooting, I could try my best to accurately recreate crashes and upload the file before it deletes itself.

Hi user365,

Could you explain what you have done to narrow it down?

Hi user365,

Could you explain what you have done to narrow it down?

For starters, the DMP file I have provided references ntoskrnl.exe as a cause behind the crash, which made me initially suspect a possible driver issue. I have since completely reinstalled my display drivers with the help of DDU. I went to great lengths to update other drivers, most of them manually. This proved to not fix my problem upon my next gaming session, so I went to the next best step (according to research I conducted).

I ran verifier.exe and selected all relevant settings. I made sure to NOT select drivers provided by Microsoft, but despite that, running "verifier /querysettings" in the command prompt showed that it was checking for SOME Microsoft drivers. Even then, I went through the tests and verifier.exe immediately detected BCCode c9 (DRIVER_VERIFIER_IOMANAGER_VIOLATION).

I rebooted around five times so I could compare DMP files, and all of them resulted in code 0xc9, which then pointed to ntoskrnl.exe with a random accompanying sys file. For example, two DMP files pointed to usbport.sys and videoprt.sys (both Microsoft drivers I didn't select but verifier decided to include anyways) respectively, and the other three I cannot remember (not at home on my computer at this moment). Point being, none of the DMP files pointed to a single sys file causing code 0xc9--all of them were different with each. Keep in mind that two out of the five pointed to third party drivers (not Microsoft).

This caused me to become suspicious that this was something more than a driver issue, such as malware, defective hardware, and/or a corrupted system.

Basically, as of right now, I have no idea what file/driver/whatever is causing code 0x3b (and that one 0x19 case).

I'm fairly positive I haven't done all I could have thus far, and this is why I'm here. If all else fails, though, I do have a contingency plan which is as follows:

I rule out all driver, malware, and system corruption issues by 'simply' overwriting my disk and reinstalling Windows. (Though it goes without saying I'd prefer not to go through the chore of recovering around 700GB of data, so that's my contingency/nuclear option.)

Here are some DMPs from the tests I ran yesterday. I'll run more tests with verifier.exe today, as well as getting the system to crash normally.

I've haven't crashed normally yet, but I have ran additional tests with verifier.exe, so you can look at these in the meantime.

Crashed again today. Here are the two files. Not sure what's going on anymore, really.

After much debugging, crashing, and troubleshooting, I believe I may have found the problem. I'll be uninstalling this nonessential driver and conducting additional tests.

The reference count of an object is illegal for the current state of the object.
Each time a driver uses a pointer to an object the driver calls a kernel routine
to increment the reference count of the object. When the driver is done with the
pointer the driver calls another kernel routine to decrement the reference count.
Drivers must match calls to the increment and decrement routines. This bugcheck
can occur because an object's reference count goes to zero while there are still
open handles to the object, in which case the fourth parameter indicates the number
of opened handles. It may also occur when the object’s reference count drops below zero
whether or not there are open handles to the object, and in that case the fourth parameter
contains the actual value of the pointer references count.

REFERENCE_BY_POINTER (18)
Arguments:
Arg1: 0000000000000000, Object type of the object whose reference count is being lowered
Arg2: fffffa8013732060, Object whose reference count is being lowered
Arg3: 0000000000000002, Reserved
Arg4: fffe0008fffe000d, Reserved

I have been finding all these seemingly corrupt drivers. When I correct these seemingly defective drivers, I find a new one needing to be fixed. So, either every driver is corrupt in a inconsistent manner, or the NT Kernel has been corrupted. It has gotten to the point of where I believe the entire system has been compromised. Whether it be by a virus or VERY bad luck. How this happened--I still do not know, nor do I think it matters anymore at this point.

Unfortunately, I believe this is a sinking ship that needs abandoning, so unless someone else can find a magic fix; I'll be formatting/wiping the disk to reinstall Windows to eliminate any problems of malware or corruption. If that still doesn't work, at least I can say it's a hardware problem. If that is the case--then I can work my way from there.

I suppose I'll ask one more thing and mark this thread as completed once I get an opinion. Should I format the disk, or should I overwrite (wipe) the disk? It's a single HDD. Basically; which method should I implement and what software program should I use to achieve that end?

HDD has been formatted and Windows has been reinstalled. No problems during installation. No problems during installation of Windows updates, essential drivers, and basic programs (such as Steam, Teamspeak 3, and Google Chrome). A system image was subsequently created to make restoring Windows less tiresome for future problems.

Marking thread as completed, if anymore crashes occur and I cannot find the problem myself, I'll create a new thread.
