Constant BSOD mostly while gaming, but also while idle

I've recently been having issues with BSOD on my system, it's been happening at least three times a day. So far I've updated graphics drivers, run memtest86+ and made sure the temps are staying low.

One thing that is consistent is that if it happens while I'm gaming, it will freeze, and if I attempt to Ctrl-Alt-Del, it says "Preparing Security Options" and then will BSOD. Not sure if that helps determine the problem.

I'm not sure how to interpret the dump files, so hopefully someone can help with the issues I'm having. Let me know if I need to add more files.

Thanks!

Just remembered, before it crashes, it has mentioned an I/O error, as well as something to do with a .dll file causing issues

Other than that all I could really say is a clean install or even try the repair disk that came with the computer if there is one. Sorry, wish I could be more help. :(
