Might still have a rootkit infection. Depending on how you formatted the driver prior to reinstalling windows the rootkit might still be lingering.

Try running this rootkit scan
How to remove malware belonging to the family Rootkit.Win32.TDSS (aka Tidserv, TDSServ, Alureon)?

Might also be hardware issues such as memory.
Run Memtest for 7-8 passes and report back the results

* Go to search - type device manager and look for any yellow warning markers
* Download and run SuperAntiSpyware
or Malwarebytes, a full scan after you update
* Run antivirus scan
* Check memory with memtest86
* Make sure that you are not over heating.
* Try in safe mode; report back if it works properly.
* Make sure PSU is working properly
* If overclocking, restore to original
* Test hard drive at company website.
*Type event viewer, in search (by start button) and look for errors that may show the cause of the problem.Look for critical errors (they have red in the left column ).
When you find them you want to look for critical errors that say app hang, app crash, or anything that relates to the problem.

ok the results.... when i got my computer worked on about a year ago ( i got my vista switched to 7).....who ever the asshole is ****ed around with it inside. and the screws were loose on the processor and harddrive and the gooey shit in the processor was bone dry (they said it was from being old/ slight over heating from time to time.) anywho after lubing/ tightening/ cleaning everything it runs like a top. all of those programs that everyone suggested worked great and all came up positive. the guys at the shop fixed the drive that wasnt reading as well. everything is awesome. thank you everyone who helped out!
