How can i used performance monitor to monitor my hdd

I think my hdd is failing sometimes when im extracting files and watching videos tha i have on my computer with
windows media player my computer just Freezez for a few seconds .I notice tha when im multitasking meaning
tha my Hdd is doing alot of reading and writing my computer starts Freezing intermittently this is been happening
alot lately ever since i think this started happpening more and more ever since i ran Dericks boot an nuke
iran 3 passes and it took about 13 hrs

So i wanna monitor my HDD with performance monitor and be able to save a log file
to check if my Hdd is failing
