Solved Intermittent BSOD, "Windows Failed to Start" upon boot

Hi guys,

For the last few weeks, I've been getting the "Windows Failed to Start" error screen intermittently when I try to boot my computer. This does not always happen, but several times on occasion. I then proceed to run Startup Repair (with a system restore), and then I am able to launch Windows when it completes.

When I log in, I get the message that "Windows has recovered from an unexpected error," the error type being "Bluescreen." I don't recall ever seeing a bluescreen actually happening; the night before I see the error the next day, my computer seems to shut down without issue.

HIDClass BSODs can mean you have a corrupted or incompatible USB device or driver connected. Since you are having problems on startup I would recommend removing all USB devices and test to see what happens.

You have a driver named MayPro.sys. This is for a gamepad of some sorts (haven't researched yet) and has caused at least one BSOD with a quick search. Remove the gamepad if its connected and/or uninstall the driver and see if that helps.

Hello Gator, thanks for the prompt response.

Yes, the Maypro driver was for a PS2 controller to USB adapter I was using, and it was indeed plugged in. I've been using it for a few years now and it hasn't ever caused me trouble before. I did unplug the adapter and uninstalled the driver to be safe.

Because the error is intermittent/infrequent, I can't tell for sure if this was my problem. If I come across the issue again, I will update with a new log. Meanwhile, is there anything else I should do or did you see an anything else that was amiss? Yes, I can boot into safe mode just fine.

Not really, all of your BSOD's pointed to the same cause. If everything is fine without it plugged in, try booting without it and plugging it in afterwards. If that helps it may be tedious but you could just unplug the controller when you turn off the PC or turn it on..
