Subsequent to receiving the usual Windows HDD failure notification, I used CrystalDisk to verify it. (Image attached).

My question is, what do my particular stats mean? I noticed the Reallocated Sectors Count is red, and the Current Pending Sector Count and the Uncorrectable Sector Count are yellow.

Does this mean that I have ALREADY lost data? Or could it be possible that some data has already been damaged? Does this mean that some of my dada was probably lost over time?

I usually backup my data once a week, but I haven't backed it up in a month. That said, as soon as I received the windows error, I backed up all of my data immediately.

Do you think my data was affected in any way?

The reallocated sectors error is indicative of an impending disk failure. You should replace this disk as soon as possible.

The only way to know whether data is lost or corrupted is to check your backups.

Do you think you can answer this using an example? I only ask because the only data I'm concerned about are my music files and I have 150 Gigs of music. I couldn't imagine playing nearly 20,000 music tracks one by one to search for any fine signs of data corruption. That would make me . I mean, as soon as I seen the warning, I successfully did an image backup of my data using Shadow Protect.
