Speaker & Microphone always set to maximum volume

My wife has Windows 7 and the speaker is always set to maximum volume. (The computer makes a loud, shrill noise all the time.) Now, it can be muted and that is what she is doing now. In addition, the problem does not exist in Safe Mode. I have no idea what process is causing this. (Is the only way to go into safe mode and copy all of the processes and kill the remaining ones one-by-one in standard mode until I get the correct one?)

I had it fixed for a day when I restored the system to a state last week. However, after the last round of updates it started again. Does anybody else have this problem? I can't find any information anywhere. Does anybody know what is going on?

Without the system information, at least the PC make and model number we can only offer general suggestions.

Not all drivers and applications are loaded in Safe Mode. It could be some program that is loaded when Windows starts that is setting the volume to maximum (look at what is in startup, such as Skype or any other media program).

Is the correct sound driver installed? or is only the generic, basic function Windows installed High Definition Audio Driver installed? If the High Definition is what is installed the first thing to do is install the correct sound driver.
