i have just started using startorbz and when i click apply to an orb it restarts explorer shows the new orb but it reverts back to the default button, any ideas ,should i do anything ie patch anything first?

Julian you do not need to patch anything, look in C:\ProgramData\Start Orb Manager\Themes and look to see if your Orb is there.
You may want to open up a copy of explorer.exe in C:\Windows with resource hacker and see if the Orbs have changed.

I'm suffering from dry eyes - too much screen time.

Found this link The Dry Eye Zone :: Blink! which is an every ten second blinking eye reminder - apparently blinking decreases dramatically when using a PC leading to less tear lubrication and sore eyes. Thought it would be neat to have this animation permanently replace the Windows Start orb as a gentle but constant reminder.

Not even sure if it can be done so asking the experts posting to this forum whether it can be.

you cannot get a transparent background with a .jpg E need a .png or .gif or .bmp I converted it and remove the white for you I hope it's good enough.
