Windows not recognizing monitor coming out of Sleep mode

First off, here is my current build running Windows 7 Home Premium 64b overclocked at 4.2:

Intel Core i5-3570K, Asus GeForce GTX 780, Fractal Design Define R4 (Black Pearl) - Current Build - cleareyes's Saved Part List - PCPartPicker

Problem: ~40% of the time when I wake my computer up from Sleep mode, it will play the "USB Disconnect" sound, and then it doesn't send a signal to my primary monitor and instead only displays on my secondary monitor OR sometimes it won't recognize either monitor and both stay black. The computer is still running, just not displaying anything. I can restart my computer, and it displays correctly again. Obviously this isn't an ideal solution.

History: I started my build with a GTX 670 at the time (before I sold it to a friend), and the problem was present. I had been connecting my secondary monitor (a VGA only monitor at the time) in through the motherboard VGA port and figured this could potentially be the issue. I didn't have a VGA to DVI-I adapter at the time so I just dealt with it. A month later I picked up the Asus GTX 780 and replaced my GTX 670. The problem still persisted (I think this is important to note as it might rule out the issue being caused by the GPU). So after some time I finally got around to ordering a VGA to DVI-I adapter for my secondary monitor. Now my secondary monitor along with my primary were connected to the GPU, where before the secondary was connecting through the motherboard VGA port. The issue is still present. Windows/GPU drivers are all up to date. Issue is present when OC'd and when at default 3.4 clock.

It would be one thing if this problem occurred 100% of the time, but since it only happens ~40% of the time it has me thinking this might to be a physical issue maybe.

Notes: Running with two DVI widescreen monitors now, but when the problem started I was using a VGA secondary monitor.

Checked the BIOS. All Power Saving features were already disabled.
