Solved BSOD when switching from single monitor to dual(monitor + TV)

Hi guys! I've been coming across an issue lately and wanted to get some insight as to what exactly is going on. I'm guessing it's the video card, and all my drivers for it are up to date, but I want to make sure.

The first time this happened was on 5/31/2014(see attached screenshot). It's happened a total of 4 times in the past two weeks and always at the same instance. I have a single monitor plugged into my video card(HD 6850) and also have my TV plugged in as well for when I want to watch movies and such. Typically I am running my PC in dual monitor mode(duplicate screens) even when the TV is off but when I turn to TV on these past 4 times, the screen flashes black as normal, but instead of both screens appearing again in tandem the TV is blank and my pc monitor now has a GSOD. I've attached another screenshot as well from 6/4/2014.

It does not happen every time but as I noted it's done this 4 times. As instructed I've also attached my zip file with all the info from DM Log Collector. Let me know if I'm missing anything else that you need. My pc specs are in my profile as well.

ks.sys belong to the Kernal CSA Library and is handled by Windows Updates, go to Control Panel > System & Security > Check for Updates. The reported driver is outdated and needs updating.

Open an Elevated Command Prompt, type sfc/scannow and hit enter.

ks.sys has also been related to ATI HDMI Audio too, and looking through your logs we find this:

Do you use the HDMI audio output to either of your displays or a dedicated sound card or onboard sound? And can you confirm which drivers your GPU is running.

Device Manager > Expand Display Adapter > Right click > Properties > Driver

Lastly, Avast Self Protection driver seems to be giving you trouble.

- I did this but there are no new updates. The only updates available are two Bing related updates and a bunch of foreign language updates, which I don't use.

- I did this and the return was "Windows Resource Protection did not find any integrity violations.

- I do not use any HDMI audio output to any of my displays. I have a separate sound card which outputs audio via an optical cable to my sound system for my TV(a sound bar). The audio I use for my PC is output from the sound card as well via a standard audio jack. As for my video drivers, I had updated recentlyonce this began going on(see attached screen shot here - https://dl.dropboxusercontent.com/u/63091802/Capture.JPG) so that appears to be updated.

- Maybe there's something else that can be done regarding this? I've never had problems with Avast before and I've had it for years, paid for it even. So getting rid of it is out of the question to be honest. Perhaps what you found is just an artifact caused by whatever the real problem is?

That said, if all of this is basically in line, things are up to date, and disregarding the Avast question... would it seem likely it can just be an issue w/ the hardware of the video card?

This was expected. Nothing to worry about there.

This is the crux of your issue. Uninstall ATI HDMI Audio through Control Panel. If you have any problems report back.

It's up to date so I can only see it's interfering with your system. If you want to keep it it's your call.

I went into the Control Panel > Programs to see if there was anything like "ATI HDMI Audio" but there wasn't. I did however find it under Sound. I Typically had three audio options; ATI HDMI Audio - which I've never used, Speakers - which is just my desktop speakers, and S/PDIF Passthrough - which is my sound system. I disabled the ATI HDMI Audio and it no longer shows in the list. Time will tell if that's all that was needed. I'm going to mark this as solved for now. Thank you for your help
