Solved Monitor "No Signal" Turns Off when Windows 7 loads

CPU: Intel Core i5-3350P Ivy Bridge Quad-Core 3.1 GHz
MoBo: ASRock LGA 1155 Intel B75 Micro ATX Motherboard
Ram: G.SKILL Ripjaws Series 2x4GB DDR3 Ram
PSU: Antec EarthWatts 500W PSU Bronze Certified
Hard Drive: Western Digital 500 GB Hard Drive
Monitor: Acer 21.5" LED Backlight Widescreen LCD Monitor

Problem: This is my wife's PC, I built it myself taking parts off of newegg and for the better part of a little over a year, it seemed to run fine. After a year, it started having heat issues (middle/end of 2013) - we tried ordering an additional fan for her case but slowly the heat issues got worse until I finally RMA'd it recently. Right before RMA'ing her card, it suddenly stopped working - when windows would start to load it would suddenly turn off the monitor and say there was "no input". Now if I started it in safe mode (with or without networking) it'd run video fine, but not in low resolution mode (it'd still not run, monitor turned off) so I assumed it was a problem with the drivers.

I tried sweeping the drivers a couple times (100% complete uninstall) to no avail, and finally just decided to wait for the new card to come in. Sadly the new card appeared to have the same problem - computer starts...no screen input as soon as windows loads. I tried googling the problem, asking friends, all to no avail, and finally decided to just format her hard drive and start over.

After reinstalling windows on her now wiped HD...it amazingly worked, perfectly. Not a single problem in sight after that...for about two weeks.

This morning she came home, turned on her pc only to find out it's doing it again - no input. Tried wiping the drivers, didn't work. Older drivers? Didn't work. Tried doing a reinstall of windows without reformatting the hard drive - didn't work. So now I'm looking at reformatting the drive again unless I can figure out what it is. If I can't find anything else, I'm going to assume either her hard drive is going corrupt (which would be strange as it's only the video driver that keeps having issues) - or perhaps the motherboard, as the video card we got is brand new.

Any tips or suggestions on what might be plaguing my wife's computer?

Update: Formatted her hard drive, reinstalled windows 7 (64 bit), reinstalled her LAN and USB drivers and such, and then got the latest drivers for her card. It now does it straight from fresh install - straight up no input after I put the drivers on.
