Problem when playing Tomb Raider

Hi everyone,
I'm having a problem when playing Tomb Raider, after anywhere between 5-30 mins of play i lose connection to my screen and it turns off. Even though my screen has turned itself off my PC stays on and have to manually shut it down with the power button. My guess is that my GPU is overheating and shutting down or something but not 100% sure, any help would be appreciated and will attach a .dmp file that i got from it a couple of days ago.

A "stop 0x124" is fundamentally different to many other types of bluescreens because it stems from a hardware complaint. Stop 0x124 minidumps contain very little practical information, and it is therefore necessary to approach the problem as a case of hardware in an unknown state of distress.
