Hi MilAnno,
Try running the GTA IV installer in Compatibility Mode for Windows XP SP2. Here's a tutorial: http://www.sevenforums.com/tutorials/316-compatibility-mode.html

Compatibility is not a global setting, it only affects the .exe whose Properties you accessed to change it. You're good to go

Make sure you update GTA IV with the latest patches. this will fix lag issues and make it run better.
