Solved USB 3.0 flash drive can only be formatted in a USB 2.0 port

On Black Friday, I bought a Lexar 64GB USB 3.0 flash drive.
Lexar S45 64GB | Staples(R)
I wish to format the drive with NTFS, and 64kB allocation unit size. I will be storing VeraCrypt file containers on this drive.
When I plug the drive into a USB 3.0 port and I attempt to format it, Windows takes about a half hour, the drive light stops flashing, and then coughs up an error saying, "Windows was unable to complete the format", and the drive is left in RAW state.
When I plug the drive into a USB 2.0 port, the drive light flashes constantly, and I can format the drive just fine.
It doesn't matter if I right-click the drive in Computer or via Disk Management.
Am I doing something wrong? It's well known that my Etron USB 3.0 facilities on this mobo are bad enough to generate BSODs, and there are no updated drivers... Is it my mobo or is the drive faulty?

Flash drive was automatically uninstalled by Windows Explorer, then reinstalled as "unknown device". This happened frequently in the past with other USB flash drives plugged into this port.

Windows did not recognize the flash drive until after plugging it and unplugging it three times in one of two front panel 3.0 ports. The other front panel 3.0 port never recognized the flash drive at all.

Conclusion: Rear panel USB 2.0 ports work fine with this USB 3.0 flash drive. There are no front panel USB 2.0 ports on my rig.
Once again, my crappy Etron USB 3.0 chip on my motherboard proves that it's junk, and my AMD Southbridge 2.0 capabilities work just fine. I'll mark this one as solved. Thanks, Megahertz07!

Download and install USB 3 drive (1.0.0.118)

USB 3.0 cables should be short as possible.
I have a USB 3.0 front panel bracket that came with a 2.5 feet cable that wasn't working properly.
After shortening it to 8" it worked fine.

That's the driver I'm presently using, MHz. My USB 3.0 cables are permanently attached to my case and are about 20cm long.

Try to repeat the USB 3.0 failed test without the extension cables, attaching the Lexar directly to the slot.

After ±11 minutes in a rear-panel USB 3.0 port, the computer screen went black and automatically rebooted.
Checking the flash drive, there's 24GB written.
I'm pretty sure at this point it's my motherboard's USB 3.0 chip. It's widely known to be faulty.
