Windows 7 icons and extensions problem

i have windows 7 x64 and i had a problem that's been annoying me for years now which is this ,i set a custom folder icons for nearly all the folders on the hd and also cause i have 27" monitor i set the view to extra large icons now everything was working fine until one day the view were set to titles so i sat it back to extra large icons and as soon as i close the folder it went back to titles again
as i said i had this problem for years now i tried to fix it but had no luck and it wasn't a breaking point for me to stop using win 7 since it didn't effect performance at all but recently i started to see another problem when i double click a pdf file windows doesn't open it automatically but ask for a program to open it reinstalling adobe reader helps but after couple of days it went back and started to have that problem with a lot of other extensions like mp3 files,rar ,zip,and html files and other extension i use for work
as i said i have tried every fix i could find but none of them worked does anyone know what could cause this ? and how to solve it ?
another thing i will be installing a new win 7 on a new machine and i want to avoid having this problem is there anything i can do?

you hav'nt changed your windows scheme by any chance, each has to be set up individually

why not set firefox as you pdf reader

no didn't change windows scheme and i don't use firefox at all
