x64 Windows 7 Pro - BSOD Every Few Weeks - Too Many Stop Codes to List

I have a problem with a Dell Optiplex 580 running Windows 7 Professional x64. Here are some of the basic specs:


Manufacture Date: 12/29/2010
Operating System: Windows 7 Professional
System Type: 64-bit Operating System
Processor: 2x AMD Phenom II X2 B55 Processor
Memory: 4GB, Non-ECC, 1333MHz DDR3 (2x2GB)
BIOS: Version A05
Hard Disk: 250GB SATA 3.0BG/s and 8MB Data Burst Cache
Display Adapters: 256MB ATI Raedeon HD 3450
DVD/CD-ROM Drives: DVD+/-RW 16x
Network Adapters: Broadcom NetXtreme 57xx Gigabit Controller
Sound/Video/Game Controllers: Realtek High Definition Audio

The PC is only about 15 months old, but started to blue screen about two weeks out of the box. The issue usually occurs overnight when the PC is idle, but not always. The crash dumps show that every two weeks or so it goes down and has to be restarted. Using the Windows Debugger tool, no less than eight different stop codes (atikmdag.sys, win32k.sys, ntkrnlmp.exe, Ntfs.sys, Pool_Corruption, fileinfo.sys , win32k.sys, rdbss.sys) are shown as possible culprits. So much for finding one single silver bullet for fixing this thing!

Dell’s chat support offered little help. After informing them that I had run the Dell Diagnostic Utility and that no apparent hardware failures had been detected, I was quickly told that I “have a corrupt OS” and that I “need to reinstall Windows.” This was before I even provided them with a single stop code or bugchecks analysis. I gave them a string of bugchecks and after about two minutes of radio silence, I was told again “corrupt OS, reinstall.”

After not getting the answer(s) I wanted from Dell, I decided to do a little more research before committing to a total rebuild (after all, if the OS is truly corrupt, it came that way from Dell, so why would I want to go about reinstalling the OS, essentially doing their job for them?). I found some advice on this forum and others, but following up on these tips have not yielded (at least for me) a definitive cause. Here is what I have done so far…

1. I have run the full Dell Diagnostic Utility (mentioned above), but have found no hardware errors.
2. I have run a full virus/malware scan with MBAM and MSFT Security Essentials (both fully updated), but found no evidence of malware.
3. I stress tested all drivers using Verifier.exe, but no BSOD occurred after reboot.
4. I ran 3 or 4 passes with Memtest86 with no errors
5. I have run a CHKDSK C: /F /R with the following results:

I am pretty much stuck as to what I should do next. The people at Dell may be correct that I need to reinstall the OS, but before going to the trouble of doing so, I wanted to rule out several potential issues before getting started on that.

The computer is up-to-date with Windows Update. It has the latest BIOS version, and as far as I can tell, all drivers are signed and up-to-date. Any help that can be provided would be great. Thanks!

Thanks for the quick reply. I instaled the Catalyst driver and saw no errors in the install log. I rebooted, then ran the SFC /SCANNOW Command from the elevated Comand Prompt. No integrity violations were found. I sure do hope that this does the trick. Would the SFC /SCANNOW command be the only way to verify that this has been fixed? The only reason I ask is because the PC only crashes once or twice a month. So in the past, I have applied what I thought might be the fix, only to have it BSOD a few weeks later. I was not, however, using the SFC /SCANNOW Command afterwards, so who knows if it would have spit out a few integrity violations. Again, thanks for your help. I will report back to let you know if anything else pops up.

Do what Jaidyn suggests first as your video driver is over two years old and System file check will give you a good indication of win 7's health.

In looking at your 23 crashes (in one day, ugh) I noticed a pattern emerging. Fifteen of them were caused be a "memory exception" (c0000005) which is probably a driver.

Given that please run these two tests


Please run these two tests to verify your memory and find which driver is causing the problem.

* If you are overclocking anything reset to default before running these tests.
In other words STOP!!!

* If you have raid update its Driver.




Memtest.
*Download a copy of Memtest86 and burn the ISO to a CD using Iso Recorder or another ISO burning program. Memtest86+ - Advanced Memory Diagnostic Tool

*Boot from the CD, and leave it running for at least 5 or 6 passes.

Just remember, any time Memtest reports errors, it can be either bad RAM or a bad motherboard slot.

Test the sticks individually, and if you find a good one, test it in all slots.

Any errors are indicative of a memory problem.

If a known good stick fails in a motherboard slot it is probably the slot.

Driver verifier


Using Driver Verifier is an iffy proposition. Most times it'll crash and it'll tell you what the driver is. But sometimes it'll crash and won't tell you the driver. Other times it'll crash before you can log in to Windows. If you can't get to Safe Mode, then you'll have to resort to offline editing of the registry to disable Driver Verifier.

So, I'd suggest that you first backup your stuff and then make sure you've got access to another computer so you can contact us if problems arise. Then make a System Restore point (so you can restore the system using the Vista/Win7 Startup Repair feature).

Then, here's the procedure:
- Go to Start and type in "verifier" (without the quotes) and press Enter
- Select "Create custom settings (for code developers)" and click "Next"
- Select "Select individual settings from a full list" and click "Next"
- Select everything EXCEPT FOR "Special Pool", "Force Pending I/O Requests" and "Low Resource Simulation" and click "Next"
- Select "Select driver names from a list" and click "Next"
Then select all drivers NOT provided by Microsoft and click "Next"
- Select "Finish" on the next page.

Reboot the system and wait for it to crash to the Blue Screen.
Continue to use your system normally, and if you know what causes the crash, do that repeatedly.
The objective here is to get the system to crash because Driver Verifier is stressing the drivers out.
If it doesn't crash for you, then let it run for at least 36 hours of continuous operation.

Reboot into Windows (after the crash) and turn off Driver Verifier by going back in and selecting "Delete existing settings" on the first page, then locate and zip up the memory dump file and upload it with your next post.

If you can't get into Windows because it crashes too soon, try it in Safe Mode.
If you can't get into Safe Mode, try using System Restore from your installation DVD to set the system back to the previous restore point that you created.

Thanks for the input! I don't know if the red font is part of your signature or specific advice to be, but there's no overclocking going on with this PC, and there's no RAID, so no driver to update. I have run Memtest86 and Verifier.exe with no errors. Since the system file check came up clean, I am hopeful that the issue has been resolved. It is strage, because I could have sworn I had updated the video driver previously, but I guess not. I hope that this will be the last I see of this issue on this PC. Thank you again for your help!

My worst fears gave come true. In the past, whenever I tried fixing this PC, Everything seemrd to run fine for about 4 weeks, when all of the sudden the PC BSODs on 3/28 and 3/29. The user received the following error report both times. I have attached the dump files. I would think that the two most recent ones are the ones needing reviwing.

My worst fears gave come true. In the past, whenever I tried fixing this PC, Everything seemrd to run fine for about 4 weeks, when all of the sudden the PC BSODs on 3/28 and 3/29. The user received the following error report both times. I have attached the dump files. I would think that the two most recent ones are the ones needing reviwing.

Still memory exceptions made worse by over 14 days straight uptime. That in itself may be why it seems ok for 4 weeks then starts crashing.
These crashes were caused by memory corruption/exception (cx05) probably a driver.
Please run these two tests to verify your memory and find which driver is causing the problem.


* If you are overclocking anything reset to default before running these tests.
In other words STOP!!!

* If you have raid update its Driver.




Memtest.
*Download a copy of Memtest86 and burn the ISO to a CD using Iso Recorder or another ISO burning program. Memtest86+ - Advanced Memory Diagnostic Tool

*Boot from the CD, and leave it running for at least 5 or 6 passes.

Just remember, any time Memtest reports errors, it can be either bad RAM or a bad motherboard slot.

Test the sticks individually, and if you find a good one, test it in all slots.

Any errors are indicative of a memory problem.

If a known good stick fails in a motherboard slot it is probably the slot.

Driver verifier

Using Driver Verifier is an iffy proposition. Most times it'll crash and it'll tell you what the driver is. But sometimes it'll crash and won't tell you the driver. Other times it'll crash before you can log in to Windows. If you can't get to Safe Mode, then you'll have to resort to offline editing of the registry to disable Driver Verifier.

So, I'd suggest that you first backup your stuff and then make sure you've got access to another computer so you can contact us if problems arise. Then make a System Restore point (so you can restore the system using the Vista/Win7 Startup Repair feature).

Then, here's the procedure:
- Go to Start and type in "verifier" (without the quotes) and press Enter
- Select "Create custom settings (for code developers)" and click "Next"
- Select "Select individual settings from a full list" and click "Next"
- Select everything EXCEPT FOR "Special Pool", "Force Pending I/O Requests" and "Low Resource Simulation" and click "Next"
- Select "Select driver names from a list" and click "Next"
Then select all drivers NOT provided by Microsoft and click "Next"
- Select "Finish" on the next page.

Reboot the system and wait for it to crash to the Blue Screen.
Continue to use your system normally, and if you know what causes the crash, do that repeatedly.
The objective here is to get the system to crash because Driver Verifier is stressing the drivers out.
If it doesn't crash for you, then let it run for at least 36 hours of continuous operation.

Reboot into Windows (after the crash) and turn off Driver Verifier by going back in and selecting "Delete existing settings" on the first page, then locate and zip up the memory dump file and upload it with your next post.

If you can't get into Windows because it crashes too soon, try it in Safe Mode.
If you can't get into Safe Mode, try using System Restore from your installation DVD to set the system back to the previous restore point that you created.

Thanks for your reply! To my knowledge, the only driver that needed updating as of a few weeks ago was for ATI Catalyst, which has been taken care of. I ran verifier.exe several times after doing so and was not able to crash the machine, even after running it for about 36 hours straight. So I'm not sure if it is driver related, unless there are other tests I can run to check for this.

I also ran several passes of Memtest86+ a few weeks ago without any errors. I did not swap out memory or change slots though, since the two cards never showed any errors in the slots they are presently set in.

I am with you 100% about the user leaving their system running for so long at a time. This is something I have mentioned to her several times, although, I have done so in reference to general performance of the PC, not as a means to reduce BSOD. The 2+ weeks of runtime means that the computer has not been turned off or restarted since I rebooted from Safe mode a few weeks ago to turn verifier.exe off. I will stress the importance of occasionally restarting your PC (I reboot my desktop before I leave work each day). I can see how running the PC for weeks at a time might help create the exception cx05 that crashed the system on 3/28. However, it crashed on 3/29 as well after running for only a day.

Are there any other methods in which I can test for a faulty driver? If this is a problem driver, verifier.exe has not been able to root it out.

OP here, just checking in on my thread. I haven't had any success in curbing the BSODs. Let me give a quick rundown of the system again.

Manufacture Date: 12/29/2010

I have received a lot of great advice in this thread, but unfortunately, I have not been able to come up with a solution. I have done extensive tests on the PC's RAM and hard drive, per the board's suggestions. I have also updated drivers on the PC, and to the best of my knowledge, they are now all up-to-date. Below is a run down of some of the testing I have performed:

*Scanned for viruses/malware using MBAM and MSE (no evidence of malware found)
*Run full Dell Diagnostic Utility (found no RAM, HDD, or other errors)
*Stress tested drivers with Verifier.exe (have let run for up to 36 hours with no BSOD)
*Run sfc /scannow (no integrity violations)
*Run CHKDSK C: /F /R (results attached)
*Run several sessions of Memtest86 with no errors (including both sticks at once, as well as individually, rotating slots)

*perfmon /report seems good (results attached)
*Used SeaTools for DOS to run diagnostic on hard drive (passed with no errors)

Everything I have checked out shows the system to be in good health, yet it’s still crashing, with it being only 3 days since its last BSOD. I am running out of ideas with this issue. I am still holding out hope that someone can point me in the right direction to fix this, but so far, I’ve had little to no success.

Also wanted to add, the PC has no RAID, so there is no RAID driver to update. Also, no overclocking going on here.

I am including the following files: the last 7 dump files (the previous 23 were included in my original post), a Systems Internals Autoruns log file, a Blue Screen View log summary of all dump files, my latest CHKDSK results, a screenshot of CPU-Z results for sytem and hardware benchmarks, as well as a benchmark summary html file, my most recent MBAM log, an HTML document listing non-Microsoft drivers (that I think are all up-to-date), a RAMMon summary of the DDR3s, and a text file with my last sfc /scannow results.

The crashes primarily point to graphics card issues.
  • First: Make sure AMD Vision Engine Control Center is not overclocking your graphics card without your knowledge.
  • Second: Try older drivers. Start with 11.9 and work your way up to 12.4; see if any provide more stability.

    Use the following method to re-install the drivers.
    1. Download the drivers you want for your display card(s)
    2. Click Start Menu
    3. Click Control Panel
    4. Click Uninstall a program
    5. For AMD:
      • Uninstall AMD Catalyst Install Manager if it is listed (this should remove all AMD graphics software and drivers)
      • If AMD Catalyst Install Manager is not listed, use the following method to uninstall the graphics drivers (this applies to onboard graphics, as well):
        1. Click Start Menu
        2. Right Click My Computer/Computer
        3. Click Manage
        4. Click Device Manager from the list on the left
        5. Expand Display adapters
        6. Do the following for each adapter (in case you have multiple display cards)
          • Right click the adapter
          • Click Uninstall (do not click OK in the dialog box that pops up after hitting Uninstall)
          • Put a tick in Delete driver software for this device (if this option is available, otherwise just hit OK) and hit OK

          Alternatively:
          1. Login as an adminstrative user
          2. Click Start Menu
          3. Click Control Panel
          4. Click Hardware and Sound
          5. Click Device Manager (the last link under Devices and Printers)
          6. Expand Display adapters
          7. Do the following for each adapter (in case you have multiple display cards)
            • Right click the adapter
            • Click Uninstall (do not click OK in the dialog box that pops up after hitting Uninstall)
            • Put a tick in Delete driver software for this device (if this option is available, otherwise just hit OK) and hit OK
      • Restart your computer after uninstalling drivers for all display cards
      • Install the driver you selected for the display cards once Windows starts


Each time you install a new driver, check AMD Vision Engine Control Center to make sure it is not overclocking the card. You may also want to try installing the ATI drivers without the Catalyst software:
  • You should already have the installer downloaded and installed the drivers/Catalyst software, so your drivers probably exist in C:\AMD\Support\xx-x_vista_win7_64_dd_ccc where the x's replace your version number of the driver software.
  • Uninstall all AMD software related to your graphics card by uninstalling AMD Catalyst Install Manager in Start Menu -> Control Panel -> Uninstall a program
  • Restart your computer.
  • Re-install your drivers from the C:\AMD\Support\xx-x_vista_win7_64_dd_ccc folder using a similar method to that in OPTION TWO of http://www.sevenforums.com/tutorials/98073-drivers-install-vista-drivers-windows-7-a.html
