Battlefield 4/nvlddmkm crash 'GetDeviceRemovedReason'

For a little while now, I've been getting this crash.


It doesn't seem to be subjective to the nvidia driver, since I've just updated it and the issue persists, but it always happens whenever the GPU reaches 100% usage. The driver will crash (sometimes more than once) at that point, and, if it's bad enough, I'll have to CTRL+ALT+Delete to close the game.

I've not experienced this enough to be sure, but after this crash, a restart is required if I want to play the game again. Otherwise, the game will hardly launch before the same crash occurs.

This only happens on Battlefield 4; other intensive games like GTA 5, while I've not seen it push 100%, have not caused crashes.

The card I have is a reference design I got from an EVGA RMA, don't think it's new. These crashes started happening after a particularly lengthy session on Battlefield 4.

Some regular troubleshooting steps:
- Run in cmd: sfc /scannow
If you get any corrupted but not fixed warnings, then run in cmd:
findstr /c:"[SR]" %windir%\logs\cbs\cbs.log >sfcdetails.txt
and attach the sfcdetails.txt

- Clean reinstall NVidia drivers with only Graphics Drivers and PhysX.

Graphic Card Driver Clean Installation (Belongs to MistUnleashed):
1.Run the Guru3D Display Driver Removal Tool: Display Driver Uninstaller Download version 15.3.0.0 Display Driver Uninstaller Download version 15.3.0.2
2.Download CCleaner here (CCleaner - Download) and run both the Cleaner and the Registry Cleaner to remove left-over keys.
3.Go into C:\Program Files & C:\Program Files (x86) & C:\ProgramData and delete any leftover Nvidia files.
4.Disable automatic driver installation: https://support.microsoft.com/en-us/kb/2500967
5.Restart Computer.
6.After restart go to: Drivers - Download NVIDIA Drivers and install the latest Nvidia Package (Including the 3D Vision Drivers - Install it all). Drivers - Download NVIDIA Drivers
7.Re-enable the automatic driver installation: https://support.microsoft.com/en-us/kb/2500967
8.Run CCleaner again (as instructed in Step 2).
9.Restart Computer.

- Update other drivers if possible.

- Re-download BF4.

Try running cmd as admin (Start Menu search box -write "cmd", right click when it shows and "run as administrator").

utc.app.json and telemetry.ASM-WindowsDefault.json are false corruption flags, known issue, they are not corrupted. If you still want to get rid of the warning, uninstall KB3068708 and KB3022345 (if still shows in Installed Windows Updates).

1. Press the Windows Key on your keyboard and X to open up the Power User Tasks Menu
2. Click (or tap) Command Prompt (admin)
3. When command prompt opens, copy and paste the following commands into it, press enter after each
   
   DISM.exe /Online /Cleanup-image /Restorehealth
   
   Wait for this to finish before you continue (approximately 15-20 minutes)
   
   sfc /scannow
   Wait for this to finish before you continue (approximately another 15 minutes)

I see similar issue at the last page of that link above, in case Dism command doesn't work, check that out.

Ran that command in your quote, however I don't know what it does/if it will resolve this issue. Here are the non-Microsoft services I run.

I don't think these are causing the issue. I haven't tried cleaning the graphics drivers yet, however.

Well run another sfc /scannow (unless you have already done after dism command) to see if corruption is fixed. It may or may not be related.

I believe reinstalling the driver has worked! So far I haven't received any crashes and I've seen it max load in-game.

You can re-try the driver re-installation, but only install graphics and PhysX drivers this time (custom install).
