Can you see the POST screen / BIOS prior to loading Windows? Or is there no signal at all?

If you can see POST / BIOS but lose signal as Windows loads - then that's interesting, as the recovery/safe mode uses the generic VGA driver.

The fact that the PC kept freezing coupled with the loss of signal suggests that either the card, or power supply may be faulty.

nv4 disp.dll is a nvidia driver, but it's not loaded or used in safemode.

This 7 mins later 'no signal', is that only in normal windows, or is does it occur it no matter where you are? ie BIOS Safe Mode etc

If it's in 'normal Windows' only - then it sounds like a simple driver issue.

If it's losing signal in the BIOS and Safe Mode (driver independent) , it may be worth checking your cards temps with something like GPU-Z if it's losing signal after a certain amount of time.

I know it sounds stupid but the loss of signal on the monitor could be a cable issue.

Not a stupid suggestion at all.

The cable could be bad, the port on the card itself, even the monitor could be kicking the bucket.
