Sequence of events:
  1. inserted new USB Flash Drive into USB port on HP 8460p notebook running Win 7 64-bit
  2. ran Macrium Reflect Back-up routine
  3. backup aborted - error message said run CHKDSK /r
  4. at Command Prompt entered CHKDSK /r - it said do you want to run CHKDSK when you reboot Y/N? I entered Y. I don't believe CHKDSK ran when I rebooted
  5. I inserted the same USB FD into a different HP 8460p notebook
  6. ran Macrium Reflect backup routine
  7. backup started and ran for over an hour but failed to complete - error message was index verification failed
  8. I removed the USB FD and rebooted the system
  9. inserted the USB FD, the light on the Flash Drive turned on, however Windows Explorer did not detect it
  10. I inserted another USB Flash Drive, the same make and model USB FD, in the same USB port; it was immediately detected and accessible.
What should I do next?

Try running the disk check with chkdsk c: / r if it runs you will know as it will take a while to run

Correction to previous post – when I entered CHKDSK C: /r at the Command Prompted and rebooted, CHKDSK ran on 6 out of 7 PCs. It did not run one PC. On that computer I tried using Macrium Reflect to make an image file of the System Partition and it failed to complete. The error message said MFT corrupt Error code = 6. Next I ran Malwarebytes; no threats were detected What should I do?
