Before I begin, this is the last WhoCrashed summary report of my BSOD. The other four entries are exactly the same, except for the obvious differences of time and date:

This was probably caused by the following module: ntkrnlmp.exe (nt!KeBugCheckEx+0x0)
Bugcheck code: 0x1 (0x74B12E09, 0x0, 0xFFFF, 0xFFFFF8800291FCA0)
Error: APC_INDEX_MISMATCH
Bug check description: This indicates that there has been a mismatch in the APC state index.
The crash took place in the Windows kernel. Possibly this problem is caused by another driver that cannot be identified at this time.
The PC is a Toshiba C655 Windows x64 with an i3 [email protected] GHz, and 4 GB of RAM. As requested in the post regarding posting instructions, I have generated a report with the information required.
Now, to summarize the issue at hand: This is my Uncle's PC, and I've been troubleshooting this remotely for the past couple of days. In his words: "I hate Blue screens. same one it came up right after all my icons appeared.. hope that bit of information helps. Also, I was trying to launch firefox, but it sometimes just happens at boot. If I hibernate instead of doing a full shut down, I don't get a blue screen"
The last thing he remembered prior to this was having some issues printing. I proceeded to research, and found that this issue may be related to a driver, and seeing as that was the last driver updated, assumed that it was the source of the issue. I uninstalled the driver, rebooted, and proceeded to get the base driver only for his HP printer. The issue still persisted.
I ran a chkdsk to test for possible hard drive corruption, and also used the Memory Check function integrated with Windows, with no errors returned. I've run ESET Smart Security 7, as well as MalwareBytes and a check with HijackThis in an attempt to see if perhaps it was a malware related issue affecting the kernel, but again came up with nothing found.
I'm scratching my head raw trying to figure this one out, and I'm at a complete loss. What little research I've been able to do has lead me down my current troubleshooting path, but gives no results, so my next idea was to submit a request for help here. I've always gotten great help from reading these forums, and I hope that now that I've registered, I'll be able to give back to a community that has kept me going for as long as you folks have, and I look forward to getting a bit of help again here today. (Also, an SF dump seems to be an exact copy of a dump using the first tool listed in the Instructions thread, however, that dump will be appended to this post within the next twenty minutes for review, should it contain data not present in the first dump.
