Have had same BSOD for the past year. Averages once a day. Sometimes many times a day. Have started down the self-troubleshooting road many times but have not gotten anywhere. Now I'm here for some professional help. Attached is SF Diagnostic Tools zip file. Thank you in advance for your help!

Hi, sorry that no one replied to your thread until now.

Your bugchecks are are all 0xFE which points to you intel USB controller.

Code:
BUGCODE_USB_DRIVER (fe)
USB Driver bugcheck, first parameter is USB bugcheck code.
Arguments:
Arg1: 000000000000000[COLOR=Blue][U][B]5[/B][/U][/COLOR], [U][B]USBBUGCODE_INVALID_PHYSICAL_ADDRESS
The host controller is [/B][/U][U][B]using a physical memory address that was not allocated by
[/B][/U][U][B]the USBport driver.[/B][/U]
Arg2: fffffa800d8fa1a0, Device extension pointer of the host controller
Arg3: 00000000[COLOR=DarkOrange][U][B]8086[/B][/U][/COLOR][COLOR=MediumTurquoise][U][B]1c26[/B][/U][/COLOR], [COLOR=DarkOrange][U][B]PCI Vendor[/B][/U][/COLOR],[COLOR=MediumTurquoise][U][B]Product id[/B][/U][/COLOR] for the controller
Arg4: fffffa800deaf258, Pointer to Endpoint data structure

First start off by updating your USB intel drivers using this online utility;

If you're not using the Bluetooth module I recommend removing it completely as it causes
issues more often than not as well as Athdfu.sys appears in the unloaded modules list
which is an indicator of activity during the time of the crash.

Your CPU is overclocked;
Code:
!sysinfo cpuspeed
CPUID: "Intel(R) Core(TM) i7-2600K CPU @ 3.40GHz"
MaxSpeed: 3400
CurrentSpeed: [COLOR=Red][U][B]3411[/B][/U][/COLOR]
Please revert any overclocked components to stock speeds.

   Note

Write down the current value of the SATA Mode!
Its either AHCI or IDE.
After resetting the CMOS go back and verify the value for SATA Mode is what it was
when the OS was installed.

Post back with the results!

Results (so far)

Thank you so much yoyo!
Have done what you recommended and no further BSOD over the past 4 or so hours.
One point of clarification... You recommended removing the bluetooth module. I simply disabled it in device manager because uninstalling driver simply results in reinstall on restart. Is that acceptable or is there something else I should be doing?

Hi :) , the bugcheck is the same.
Did the online utility find anything to update, because I can't see a difference.
Also your CPU is still overclocked, did you follow the steps detailed in the link above to reset it back to stock?

What kind of hardware do you have connected via USB?

Hi yoyo
I did everything you suggested
1. Ran the Intel® Driver Update Utility and updated the network card driver
2. Uninstalled the BT driver and when it reinstalled I disabled it (that was the question I posed previously... is that what you wanted me to do?)
3. And reset my bios to default which brought my CPU speed down to 3400. Not sure why it is running faster again
The only thing I have connected via USB is the data cable to my UPS Backup

If you just have the cable connected to your machine remove it, there could be a problem with the cable or the USB socket its connected to.

If you have the USB device connected, try running without it, remember to "safely remove".

Short answer, there may be a problem with the socket, try a different connection, anywhere that will allow your USB drive to work, but not the one next to the one you took it out of.

Your welcome, don't be alarmed if some of the front USB sockets won't work. Their not designed to run large devices.

You try updating Renesas Electronics USB 3.0 Host Controller Driver (c:\windows\system32\drivers\nusb3xhc.sys). The installed driver is very old there.
Code:
fffff880`049c7000 fffff880`049f8000 nusb3xhc (deferred)

The website of ASUS P8P67 PRO does not contain a feasible update there. Try in Station-Drivers - Renesas/Nec.

And, what have you done with the Atheros BootROM that YoYo155 suggested earlier?

Would you please be specific re which Renasas driver to download? There are many options on the page you linked to
As I mentioned previously, I uninstalled the Bluetooth driver but it just reinstalled on restart so I disabled it. Is that what you suggest?

Hi Arc
Took a chance and installed Version 3.0.23.0 which appeared to be the latest
No problem running the update but on device manager still shows version 2.0.32.0 from 2010
Did I pick the wrong one?

Well, good to know :) And wish that the disabled bluetooth will keep the Boot Rom away. If you upload your MSINFO32.nfo file., we can check for it.

  1. Click on the start button
  2. Type "msinfo32" (without quotes) in the search bar of the start menu, click the resulting link. It will open the System Information window.
  3. File>Save. In the "File Name" filed, put "MSINFO32" (without Quote), give the save location to desktop, and click the "save" button.
  4. Give the time for processing, it will save a .nfo file on your desktop.
  5. Zip it, and upload it following the instruction.
And let us know how it is running :)
