The preview pane only works on file thumbnails, it does not work with ico files. Check that your computer has thumbnails enabled. In Folder Options > View Tab, the checkbox Always show icons, never thumbnails should be unchecked.

I can see previews for icon files in my Preview Pane.
I know these files contain multiple icons (i.e. different sizes of the same icon).
Note:
I have installed a Shell Extension called IconViewer (v3.02).
It may be allowing me to see icon previews.

You're correct, if this option is checked I don't see any previews in Windows Explorer.
