I use Windows 7 Ultimate 64-bit with a GTX Titan. Triple displays.
I was playing a game and got a weird graphical crash during a cutscene. All three monitors went blank. (each a different color which was odd) I had to forcefully shut down the computer as I was never able to move past this. It didn't seem like I was able to interact with the computer at all with keypresses.
So after rebooting, the computer was ok until I tried to play the game again. Once again, during the cutscene, the same thing happened. On the third time, when I went to boot the computer up, during bootup, it would do everything normally, including the Windows startup animation. But then a blank screen. It never went to the login prompt. And trying to type my password didn't help.
I shut down and went into safe mode. My files and drives were all fine. I tried uninstalling my graphics driver, then when I would reboot and reinstall my nvidia drivers, I'd get the same issue where it was a blank screen instead of the login screen.
To rule out any issues with the monitors, I unplugged two of them, but then when I booted up after a hard shut down, rather than having the option to enter safe mode, I only got the options to start normally (which fails) and windows repair, which just goes through a long drawn out repair and fixes nothing.
I plugged the monitors back in and am still unable to even get the safe mode option anymore.
