BSOD possibly from Nvidia nvlddmkm.sys Driver

Ok, it seems I posted this on the wrong forum originally, as I had to recreat my account there, but this site had no record of my posts on this issue. BSOD possibly from Nvidia Drivers | Windows 7 Forums

Anyway, I was playing Star Trek Online the other day, and suddenly my PC completely froze up. Mouse and keyboard would not respond either. I hit the reset button and when it tried to start up I got BSOD, mentioning nvlddmkm.sys. The computer will only start up in safe mode now. I tried running disk check, which took over 6 hours and found nothing, and I tried the Windows Memory Diagnostic tool, which made it to 21% and then made no further progress in over an hour. I have since run it to completion and it found no issues.

I also tried uninstalling my Nvidia drivers, but have had numerous issues with that, telling me certain ones cannot be uninstalled in safe mode, specifically the PhysX driver. I downloaded and tried to install latest driver for my card, a 560TI, but says that it cannot install in safe mode either.

I tried to run Speccy, and it cannot access all of the computer components either.....CPU-Z also encounters an error on start, and won't display my memory cards.
I ran MemTest from a disk for 5 passes, which took over 6 hours and no errors were listed.

I'm not sure what had caused all this, I have not updated my drivers in the last few weeks, since before the holidays, and not had an issue. The only thing new with this PC is the mouse. My Logitech G500 starting turning on and off repeatedly, in all USB ports. I tried a friends HP mouse and this issue stopped, so I doubt it was the USB ports. Last week I bought a new Logitech G502 Proteus Core and haven't had any issues with it so far.

After disabling the Graphics Cards Adapter Driver, as shown in the second attached picture, my computer is able to start up in normal mode. However, even after installing the latest drivers for my card, I get the BSOD again, because it re-enables the above driver....

Does this mean my video card is dying?

I should also note that I have run MalwareBytes and SuperAntispyware in safe mode multiple times, and neither have detected anything except tracking cookies. However, all of my System Restore Points are gone....which suggests it could have been triggered by a virus or malware, but I don't know for certain.

The link at the beginning of the post includes all information I provided before, including the last 5 mini memory dumps from 1/27/15. I have not updated them since then, because I am having the same issue....I'm not sure if they are needed or if they are included in the DM Log Collector zip file..... but the mini dumps appear to be too large for this forum....

The most recent Nvidia drivers have been a total disaster. Just check their forums. Its a nightmare. Just use a program called Display Driver Uninstaller or DDU and it will totally wipe any and all remnants of your display driver, then reinstall the last good one for your setup.

Ahh, well the thing is, I think I was still using the previous drivers when this all started, version 359.06 vs. the newest one 361.75. I also noticed last night that the Sleep and Hibernate options are greyed out under the Shut Down options.

I will give DDU a try and see if that works, thanks bitplayer.

Unfortunately, DDU does not seem to have worked. I ran it successfully, and started up in Normal mode. I proceeded to install an older Nvidia driver, 359.00, but upon restart, I got the same BSOD as before... And once again Sleep and Hibernate are disabled

I'm at a loss again as to what to do.

Also, I noticed that after I ran DDU and reinstalled Nvidia driver, the computer reinstalled 4 Audio Drivers that all had the same name....


Also is an image of BlueScreenView. It seems to be flagging 3 files in red...

Update: Today I ran DDU again, restarted, ran it a second time. Then ran Windows Startup Repair.
Next, I tried installing the Nvidia drivers that came with my graphics card from the disk. The computer immediately started to get the same BSOD upon trying to restart...

So again I'm back to square one here, I can only run the computer in safe mode or with the Display Adapter driver disabled, all other drivers do not seem effecting start up as long as the Adapter is disabled.

Also, I just noticed in your first BSOD pick there is pretty serious artifacting going on there on the screen. Are you getting that regularly? That could for sure be a sign of either an improperly installed or dying GPU.

No, I haven't tried yet. I'm hoping I wont have to, but I have backed up my files in the event that I do. However, I'd like to know if my Gpu is failing first, if so I'll just buy a new one and hopefully not have to wipe everything.

The first BSOD pick was taken with my iPhone, the computer itself doesn't look like that.

Just tried to install and run Furmark, it says "Could not initialize ZoomGPU. Furmark startup failed. Bye!"


Also looking at BlueScreenView, it says that all of my BSOD were caused by dxgkrnl.sys, address dxgkrnl.sys+5d134. And dxgkrnl.sys, dxgmms1.sys and nvlddmkm.sys are all in red.

If you have a partition on your hard drive you can use, just do that. Just install windows on a separate partition and see if it works. If it does, just wipe the other (your current) install and bring all those backed up files over.

My opinion would be to just go for it.

Your stuff is backed up. Windows takes what? 25 min to install if you make a bootable usb? Go for it.

Here is DM Log, I did notice that it failed to copy the Mini dump file because it couldn't find it for some reason....I did recently run CCleaner again, maybe it deleted it?

It seems there are no dump files, however last few crashes are 0x116 while others are 0x7A and 0xF4 which usually point to hard drive problems.
Please follow below instructions carefully.

Please uninstall AVG while troubleshooting and replace it with MSE

Daemon tools, Alcohol 120% and Power Archiver Pro uses SCSI Pass Through Direct (SPTD), which is a well known cause for BSOD's.
Please remove Daemon Tools and run the SPTD pass through remover.
  1. Uninstall Daemon Tools
  2. Download the SPTD standalone installer and follow these steps:
    • Double click the executable to open it
    • Click on the button shown below

If the button is grayed out, like in the image, there is no more SPTD installation on your system, and you can close the window





Diagnostics Test

 System File Check


Please try following:
  • Open an admin command prompt
  • Copy/paste "sfc/scannow" (without quotes) and press enter
  • When it finished reboot your system
  • Open again an admin command prompt
  • Enter sfc/scannow again
If sfc/scannow does NOT say "Windows Resource Protection did not find any integrity violation", please upload the cbs.log file located at {windows partition}\Windows\Logs\CBS\, if the file is too large try a 3rd party uploader like dropbox, onedrive, google drive, mediafire etc.
System File Check(SFC a.k.a. Windows Resource Protection) needs to have your system rebooted in order for sfc to try to fix the problems that it finds.

Run following tests
  1. Short Drive Self Test
  2. Short generic
  3. Long generic
If the short generic fails, no need for the long generic.

   Note
Do not run SeaTools on an SSD as the results will be invalid.

  • scan for errors, no quick scan but full scan
  • check the health,
  • benchmark.
It may take some time, but please take the time you need to perform it properly.
Let me know what the results are
  • of the error scan,
  • make a screenshot of the health of every hard drive and post the screenshots,
  • post screenshots with the benchmark of the
    • transfer rate,
    • access time,
    • burst rate,
    • cpu usage.

Currently running Seagate tests, hard drive passed the 2 short tests, still working on the long test. However, the program kept showing errors regarding logs.

SPTD didn't detect anything after removing Avg and Daemon tools. System file check didn't detect any issues either.
