Just ran across this thread from a google search after having experienced this exact problem now for a few weeks. I finally isolated, by trial-and-error, the culprit, at least in my specific circumstance. The problem program is KeyScrambler (paid Premium version in my case), a security program designed to help prevent hidden, malicious keystroke logging. I'm not sure how long it's been an issue with the program versions, only that it still exists of the most current version (2.8.2.0). I've contacted the maker via email with the details so hopefully some future release will address it effectively.
The only solution that has worked for me so far is to either 1) unload the program from memory (using the option to just turn it off but leaving it memory did not work, the renaming of a folder with the save-as dialog window from either IE[9] or Firefox 6.x still failed with the folder staying with the default "New Folder" name) or 2) from the Options>Advanced tab, uncheck or de-select "Protect Windows Explorer [Premium]". Either of these 2 solutions corrected the problem so that clicking the "New Folder" button on the save-as dialogs resulted in a correctly renamed folder location.
Anyway, just thought I'd update this old thread with my findings in case anyone else runs into the same problem and has KeyScrambler [Premium] installed on their system.
